Have you ever heard about a year based day program?

Resilient Hearts offers individualized life skills day program for individuals to empower them to be self reliant by providing hands on life skills and self advocacy education. Our schedule is based on our ten guiding principles that help us to plan their care and our supports.

Eligibility 

  • Participants age should be 16 years of age or older.

​​

  • Participants must identify themselves with disability.

​​

  • FMS (Family Managed Supports) through PDD or FSCD funding.​

​

  • AISH or Private Funding is also an option.

​​

  • Participants must have a valid government-issued ID.

​​

  • Participant must agree to participate in the program voluntarily.
